Your initial visit
Your first visit will consist of a detailed health history and physical examination to determine if your condition is appropriate for treatment. Dr. Hemsley will discuss your condition with you and establish a treatment plan based on your individual needs and goals.
Please arrive 10-15 minutes prior to your scheduled appointment to fill out some brief paperwork. New patient paperwork is provided below if you would like to fill it out prior to your appointment. If you have insurance that covers chiropractic, please contact your carrier to verify eligibility, co-pay and amount of your deductible if appropriate. Remember to wear comfortable, loose fitting clothes.
